The Philippine Consulate General in San Francisco will conduct a consular outreach mission in Seattle, Washington on June 22-24, 2012.
When: June 22, 23 & 24, 2012 (Friday, Saturday and Sunday)
Time: beginning at 8:00 a.m.
Where: Filipino Community of Seattle (FCS) Center

Services to be rendered:
Passport Services: Passport applicants are no longer required to bring photographs since they will have their pictures taken using the ePassport data capturing machines at the outreach site. However, they still need to bring other requirements, such as their current passport, the passport application form (which may be downloaded from the Consulate website - ) http://www.philippinessanfrancisco.org/uploads/pdfs/passport.pdf) , an application fee of $60.00 and a self-addressed stamped (Express Mail with tracking number $19.00) envelope or a prepaid courier envelope, with their application. Please submit photocopies of the supporting documents (e.g for passport applications submit a photocopy of the passport data/photopage).
Dual Citizenship: Please accomplish the application form (one set with photo) plus the supporting documents. Please submit a photocopy each of the document and the ID. Fee is US$50. Notarial/Legalization of Documents: Bring Please bring the document and an original valid photo ID w/ signature (e.g. State ID), and a photocopy each of the document and the ID. Fee is US$25 per document.
Report of Birth: The birth of a child in the US should be reported/registered with the Philippine National Statistics Office through the Consulate General. See http://www.philippinessanfrancisco.org/uploads/pdfs/birth.pdf. This is needed if you wish to apply for a Philippine passport to a child born in the US of Filipino parent/s. Please prepare 5 sets of the completed Report of Birth form and required documents.
Report of Marriage: Marriages between a Filipino and a foreigner must be reported/registered with the Philippine National Statistics Office through the Consulate General. See http://www.philippinessanfrancisco.org/uploads/pdfs/Report%20of%20Marriage.pdf. This is needed if you wish to apply for a passport using your husband’s surname for the first time (female applicants married to foreigners). Please prepare 5 sets of the completed Report of Marriage form and the required documents.
